Dental visits are by Appointment Only.
If you have a reserved appointment time and an emergency arises, please kindly inform us at least 48 hour in advance, so there will be no last minute change fee. Please call us at 773-736-1406.
ALL patients deserve the best dental treatment regardless if one has dental insurance or not. All patients are treated the same, with high standards and the financial obligations are the same.
Financial Obligation for All Patients: Payment-in-full is due for services the day treatment is rendered.
Payment Types: Cash, Check, Visa, MasterCard, Discover, American Express and CareCredit.
CareCredit is promotional financing made easy! This credit card can only be used in health care settings in over 230,000 locations nationwide (such as animal & pet care, cosmetic surgery, day spa, dermatology, hearing, vision, etc). At Parkview Dental Care, fee's $200-$999 can have 6 months No Interest. Fee's $1,000 and over can have 12 months No Interest. CareCredit is subject to credit approval. For Individuals WITH Dental Insurance:
Parkview Dental Care is an out-of-network PPO provider. Parkview Dental Care is fee for service. All PPO dental benefits will be reimbursed to YOU, instead of the dental office in about 2-4 weeks. Payment-in-full is due on each dental visit.
